In order to complete summer camp registration a 2020 Camper Information and Release Form and Health History Form must be completed for each child. Camp forms may be found at http://www.decaturga.com/cys on the Summer Programs page.
Camp opens at 8:00 am and closes at 4:00 pm. After camp care from 4-5 pm or 4-6 pm may be added in the 183011 and 183012 sections below. For details on each camp session please refer to the Summer Camp Playbook at www.decaturga.com/playbook.
